BIRTH CERTIFICATE (REGISTRATION OF CHILDREN BORN OUTSIDE ZIMBABWE).

REQUIREMENTS for Registration of Children Born Outside Zimbabwe

1.  Child’s Zambian birth certificate (if applicable)
2.  Birth record / Health card
3.  Mother’s Zimbabwean ID, Birth Certificate and Passport
4.  Father’s Zimbabwean ID, Birth Certificate and Passport
5.  Marriage Certificate (if applicable)

N.B:  The forms obtained from the Embassy should be submitted to the Registrar of Births and Deaths in Harare at Makombe Building.   A payment is made upon submission of the forms in Harare.
